You would have received an invitation to the portal when you first booked your very first appointment. If you don’t have it and the system doesn’t allow you in, please text me at 614-401-6363 and I’ll resend you the invitation.

STEP 2: MANAGE YOUR APPOINTMENT

Upcoming appointments will be in blue. Past appointments will be in green.

MY MEDICAL HISTORY:

Click to update your medical history at any time.

ACTION REQUIRED:

Find pretreatment instructions to prepare for your appointments. Fill out pretreatment questionnaires and consents.

CANCEL OR RESCHEDULE:

First, click on the blue upcoming appointment square in your profile. That will take you the screen below called APPOINTMENT DETAILS. This is where you will have the option to cancel or reschedule.
